Update your details: firearm certificate holders
If you hold a firearm or shotgun certificate, you need to tell us if your details have changed.
You can use our online form to tell us about a:
- change of name
- change of address
- change of contact details (email or phone)
- change of GP
- change of GP's address or contact details
- change in where you store your firearms
To tell us about any changes, update your details online.
Medical diagnosis
You also need to tell us if you've had a relevant medical diagnosis.
The relevant medical conditions are:
- acute stress reaction or an acute reaction to the stress caused by a trauma, including post-traumatic stress disorder
- suicidal thoughts or self harm, or harm to others
- depression or anxiety
- dementia
- mania, bipolar disorder, or a psychotic illness
- a personality disorder
- a neurological condition: for example, Multiple Sclerosis, Parkinson’s or Huntington’s diseases, or epilepsy
- alcohol or drug abuse
- any other mental or physical condition, or combination of conditions, which you think may be relevant
